Life Stage Transition 0106
We provide assistance in coordinating or managing life stage transitions and supports for NDIS Participants. The assistance provided is to implement your NDIS plan strengthening independence, connection and participation within the broader community in Sydney.
Assist Personal Activities 0107
Assistance with daily personal activities with your NDIS Plan. We provide caring companionship, we will get you safely to your activity It covers support with everyday personal activities that assist you at home and in the community.
Innovative Community Participation 0116
Constellation Care Sydney provide assistance in coordinating NEW innovative goals and supports, within your NDIS Support Plan or by expanding goals to add to your current NDIS plan.
Innovative Community Participation is Assistance for NDIS participants to engage in NEW activities improving connection and participation within the local Sydney community.
Participate in Community 0125
Participation in community, social and civic activities. Assistance with Social and Community Participation within your NDIS plan, under NDIS Core Supports.
NDIS funding covers the SUPPORT, NDIS participants need to meet goals relating to participating in community, social or recreational activities. Activities may be provided in a centre or in a community setting.
Group Centre Activities 0136
We help NDIS participants in group and community-based activities and events, such as group outings or social activities at a community center. The goal to enhance social connections, promote inclusion, and develop new skills for NDIS Participants. Group Centre Activities depend on your preferences, they may include arts, crafts, sports and music. Make new friends, improve your overall quality of life.
Household Tasks support is designed to help NDIS participants manage their home. Household Tasks refers to the support provided to NDIS Participants who require assistance with everyday household activities to maintain their independence and live comfortably in their own clean safe home. This NDIS Support Service covers a wide range of activities, including cleaning, laundry, shopping, basic meal preparation, other household tasks. SEE MORE
